I confirm that I have read, understood and agree to the Creating Clarity Collective Coaching Agreement.

I explicitly consent to Creating Clarity Collective processing information I choose to provide about my ADHD, neurodivergence, physical or mental health and wellbeing where this is relevant to providing and personalising my coaching. I understand that I can withdraw this consent at any time by contacting amanda@creatingclaritycollective.co.uk.

Starting coaching within 14 days?

Please tick the box below only if you would like your coaching to begin within 14 days of entering into this agreement. If your coaching starts after 14 days, leave this box unticked.

I request that my coaching services begin during the 14-day cancellation period. I understand that if I cancel during this period after coaching has begun, I may be required to pay for the services already provided up to the point of cancellation.
